Lawful Presence Forms

As a result of a law change to O.C.G.A. § 50-36-1, the Georgia Soil and Water Conservation Commission (Commission) is required to obtain a signed and sworn affidavit verifying the applicant’s lawful presence in the United States accompanied by a copy of at least one “secure and verifiable document.” O.C.G.A. § 50-36-1(e).
Below is a link to this Memorandum.  There is also a Checklist for Verification of Lawful Presence within the United States (Checklist) along with three different affidavits from which the applicant must choose that provide the basis for his or her lawful presence in the United States. The Checklist must be completed by an applicant to identify which of the three types of affidavits and which one of the 16 secure and verifiable documents listed is being submitted to the Commission.
WHAT TO DO:
1.)  Read this FIRST:

2.)  Fill out this form: Checklist
3.)  Then based on the answers on the checklist, choose from one of these forms:
       a.) 
US Citizen Affidavit
       b.)  Qualified Alien or Nonimmigrant Affidavit
       c.)  Legal Permanent Resident Affidavit
4.)  Copy your Driver's License or other authorized document
5.)  You are ENCOURAGED to send the notarized documents to the commission BEFORE your class, however, if you do not, our trainers are all    
Notaries and can notarize your documents at the class.  Sending them in before the class will save time however, and would be appreciated by all.
